Usage of gels and films for treatment of inflammatory diseases of mucous membrane of oral cavity and periodontitis

Abstract

The work is dedicated to analysis of special literature of the recent years improving technology of the treatment of inflammatory diseases of mucous membrane of oral cavity and periodontitis. The amount of publications that are dedicated to clinical use of new medicines for treatment of these diseases increased a lot. This is due to the success of polymer chemistry and introduction of new types of medicines – dental gels and films, extension of validity achieved by immobilization of active substances on a variety of polymeric carriers. These tools of the local action provide local and even release of active substance from medical form, creating its high therapeutic concentrations in areas without a significant increase of concentration of medical substance in the system of circulation. Theoretically the film is ideal for use in dentistry. After all, being aware of all phases of inflammatory process and having a multilayers film in which every layer with necessary medicine dissolves within a specific time, we can assume that a single application of this film in a few days can be enough to eliminate the inflammatory process.

We reviewed and analyzed available soft medical forms for dentistry in the forms of films and gels used for the treatment of inflammatory diseases of mucous membrane of oral cavity and periodontitis. Besides, the authors presented their own researche on this problem.

Gels are soft medicines of local use, which are one-, two- or multiphase systems with liquid dispersive medium rheological properties, are due to the presence of low concentrations gelatinizators. Gelatinizators can also act as stabilizing disperse systems: suspensions or emulsions. The gel consists of one or more active substances and excipients that form the basis.

Dental films are hard or soft tools of proper size and forms that are designed for fixing on a particular area of the mouth. They consist of a matrix which includes the active substances or active substance is surrounded by a membrane that controls the speed of release.

At the Department of Therapeutic Dentistry of Danylo Halitskyi Lviv National Medical University for a long time was made researches to create long-acting medicines including gels and films. In recent years dental films successfully passed clinical trials, the composition of which was worked out at the department of Drug Technology of Lviv Medical University.

Dental films with Decametoxine and Chlorhexidine show a pronounced bactericidal activity against staphylococci, streptococci, fungi, viruses and some protozoa, film with Heparin activates the fibrinolytic system, eliminates stasis of blood in the microvasculature, accelerates blood circulation, reduces swelling, promotes tissue regeneration. Dental film with Amizon has a pronounced anti-inflammatory, erferonogenie and analgesic properties. Dental film with Mexidol that inhibits free radical processes, is a membrane protector, has antihy, anti-inflammatory and anti-edema effect.
