Abstract. Benign prostatic hyperplasia (BPH) is a common disease among men, especially older men, presenting with symptoms of lower urinary tract (LUTR). Modern minimally invasive surgical treatments, such as transurethral resection of the prostate (TURP), prostatic artery embolization (PAE), holmium laser enucleation of the prostate (HoLEP), aquablation, temporary implantable nitinol device (iTind), and transurethral microwave thermotherapy (TUMT), offer effective alternatives with less trauma compared to traditional surgery. Аim. To systematize scientific data on modern technologies and approaches to minimally invasive surgical treatment of BPH and evaluate their effectiveness, safety, and impact on quality of life, including preservation of ejaculatory function. Methods. Scientific sources from the electronic databases PubMed, Scopus, Web of Science, Cochrane Library for the period 2018–2026 were analyzed regarding the clinical results of the main minimally invasive surgical technologies for the treatment of BPH using the information search method, theoretical analysis, and content analysis. A systematization and comparative analysis of international clinical recommendations was carried out (European Association of Urology, American Urological Association, National Institute for Health and Care Excellence). Results. Minimally invasive methods for the treatment of BPH have significantly expanded the capabilities of urological practice, offering effective and safe alternatives to traditional surgical interventions, such as open prostatectomy. Transurethral resection of the prostate remains the reference method for prostates up to 80 ml in volume. Minimally invasive surgical techniques such as HoLEP and aquablation have been shown to be highly effective for the treatment of large BPH, while iTind and prostatic urethral lift (PUL) are optimal for younger patients with smaller BPH. Conclusions. Recommendations are offered for the selection of minimally invasive surgical techniques based on clinical symptoms, BPH size, and patient needs.
